This is an exciting full-time position to assist clients in varied transitions as there is need, from engagement in treatment to decreased level of care needs. Care coordination within these transitions to include coordination and collaboration within our integrated medical services. The candidate for this position must possess a passion for client engagement, ongoing assessment coordination and linkage and have great customer service skills.
Requires a Bachelor’s degree, and license is preferred. AoD experience, Youth and Adult experience, completing DA Assessment and provide intervention. This position requires access to an operational vehicle, valid driver’s license and auto insurance that meets SCI standards. Must be able to pass background check and drug screen.
