| Position Description: | This is a full-time position under the clinical supervision of the Associate Medical Director (AMD) for Specialized Adult Services (SAS), and the administrative supervision of the Program Manager for Specialized Adult Services in consultation with the Integrated Care Practice Manager (ICPM). The Assertive Community Treatment (ACT) Nurse Team Leader functions as part of a community based trans-disciplinary team serving adults with severe and persistent mental illness not reached by traditional services. The general responsibilities of the position are to provide coordination of daily ACT team activities, coordination of daily medication administration services and provision of pharmacological services to help clients achieve their personal recovery goals. |
| Minimum Qualifications: | Associates or Bachelor’s degree required, candidate must hold their R.N. licensure. This position requires access to an operational vehicle, valid driver’s license and auto insurance that meets SCI standards. Must be able to pass background check and drug screen. |
