Position Description:

The Wraparound Facilitator is responsible for servicing youth ages 5-14 that have an emotional, social-emotional, behavioral or mental disorder for at least one year and some form of current or past juvenile justice involvement. The Wraparound Facilitator is responsible for direct clinical intervention to the youth and the family members, as well as developing, organizing and facilitating Tiered Development Teams utilizing the Milwaukee Wraparound Model and Trauma Informed Care Model to meet the multiple needs of the children and families served.

The Wraparound Facilitator fulfills four unique roles:
• Completing/Coordinating services such as assessments and treatment.
• Crisis Intervention/Safety Planning for the youth and family.
• Care coordination through a Tiered Development Team (TDT) approach.
• Coordinating services across the multitude of systems with which the individual and family is involved.

The successful candidate should demonstrate excellent time management skills, communication skills, ability to work independently in the community, the ability to work with diverse populations, and the ability to work in a larger team.

Minimum Qualifications:

The successful candidate will possess a Bachelors Degree in Social Work or related field. Ohio licensure to provide mental health and alcohol and drug services required, LSW Preferred. Valid Ohio driver’s license required. Must be able to pass a background check, motor vehicle check and drug screen. Salary is commensurate with experience.
