| Position Description: | Full-time position functions as part of community-based, transdisciplinary team service adults with severe and persistent mental illness not reached by traditional services. The general responsibility of the position is to provide case management interventions helping clients achieve their personal recovery goals. |
| Minimum Qualifications: | Licensed as LPN with Associates or Bachelor’s degree with experience in a mental health setting preferred. Access to an operational vehicle and a valid driver’s license also required. Must be able to pass a background check and drug test. |
