Position Description:

Function as a community based case manager on a multi-disciplinary team to facilitate the recovery of the dual-diagnosed SMD population.  The general responsibilities of the position are to provide continuous stage appropriate case management interventions to help clients achieve their rehabilitation goals, and to inspire them to develop and pursue their personal recovery goals.

Minimum Qualifications:

Associates or Bachelor’s degree in the Human Services field prefer a license through the Ohio Chemical Dependency Professionals Board. AOD experience preferred.  Valid Ohio driver’s license required.  Must be able to pass a background check, motor vehicle check and drug screen.
