| Position Description: | Interested in working part time on an as needed basis? Being a part of a dynamic team? Then you are the right person. We are looking for a self motivated individual to provide back-up for multidisciplinary team providing community-based recovery services to severely disabled adults. This person will be called upon on an as-needed basis. Serve the SMD/SPMI adult population by closely monitoring the continued treatment, taking of prescribed medications and assisting these individuals in the recovery process. This includes developing mutual working treatment plans, and assisting clients in reaching treatment plan goals. The adult care specialist will demonstrate time management skills, ability to work independently out in the community, and the ability to work in as a team. |
| Minimum Qualifications: | Associates or Bachelor’s Degree in Human Services, social work, mental health technology with prior work related experience. Ohio (LPC/LSW) strongly preferred. Valid driver’s license and reliable transportation and must be able to pass a background check, motor vehicle check and drug screen. |
