| Position Description: | The individual will work closely with the Community Health Centers to support the care and to coordination with Medical, Dental and Behavioral Health providers. This position will provide health assessments and to support health and medical care for all ages. This is a part time position working 20 hour per week. Specific duties include engagement activities, screenings tools, prevention and education activities and referrals to behavioral health services as appropriate in the community. |
| Minimum Qualifications: | Master’s Degree in Counseling or Social Work and must be licensed by the CSWMFT board of Ohio or as a Clinical Psychologist. Independent License is preferred. Must be able to pass a background check, motor vehicle check and drug screen. |
