| Position Description: | This is a full time position the ACT/IDDT Nurse Team Leader functions as part of a community based trans-disciplinary team serving adults with severe and persistent mental illness not reached by traditional services. The general responsibilities of the position are to provide coordination of daily ACT/IDDT team activities and to provide nursing and case management interventions helping clients achieve their personal recovery goals. Provide coordination for the daily operations of the program, such as, leading daily morning team meetings, coordinate assignments of daily duties for the teams, and coordination of service delivery through out the day. Provide supervision and coordination to the nursing staff on both ACT and IDDT teams. |
| Minimum Qualifications: | State of Ohio license as a Registered Nurse is required. Experience with community based treatment. Must have AOD and CDR experience, Medication Management and be able to go for Home Visits and Provide Interventions. Must be able to pass a drug screen and background check. Must have a valid driver’s license and car insurance with limits of liability 100k/300k. |
